In [#60 Darkling], Robert Picardo plays Voyager's EMH and also a darkling being, accidentally created as the result of unforeseen linkages between add-ons to his personality subroutines. The add-ons are the positive aspects of historical figures of such stature as Mahatma Gandhi, Lord Byron, Madame Curie and the Vulcan T'Pau. But the darkling EMH is made up of what Torres calls the "dark threads" that also ran through their personalities.

The darkling Doctor mutilated holocharacters of the famous historical figures as he tried to extract the information he needed in order to delete the original Doctor so that he (the darkling) could survive.

In [#82 Message In A Bottle], Robert Picardo plays the EMH Replacement Program which Kim installs to try and replace the Doctor whom Paris fears will remain in the Alpha Quadrant leaving Paris to replace him permanently as medical assistant. Paris, who prefers to fly starships and his duties as conn officer, is desperate to get away from the boredom of Sickbay. At that point Paris is having to treat a number of incidences of heartburn rather than any medical emergency. Kim warns Paris that it took the finest engineering minds in Starfleet years to create the EMH (it was actually pioneered by the brilliant Starfleet holographic scientist, Dr Lewis Zimmerman, seen in [#144 Life Line]), and that he, Kim, cannot do it, but he agrees to try at Paris' behest. Kim downloads the whole of two medical authority texts into the Replacement Program, namely Gray's Anatomy and Dr Leonard McCoy's Comparative Alien Physiology. When Kim brings the EMH Replacement online, Paris suggests giving him more hair, but Kim wants to deal with minor details such as grooming later. Kim warns that creating a replacement EMH is more than just giving it a database as he would also have to create, for instance, tactile protocols so that he can perform surgery. The EMH Replacement starts to recite Gray's Anatomy, but its program suffers an overload of data, fritzes and then goes offline permanently.

In [#91 Living Witness], Robert Picardo plays two roles:
(1) a backup module of USS Voyager's EMH
(2) an evil android and a crewmember of the Federation Warship Voyager, which is the Kyrian representation of what they believe Voyager's EMH was like

In [#120 and #121 Equinox] Robert Picardo plays USS Voyager's EMH and USS Equinox's EMH. The Equinox crew has deleted their EMH's ethical subroutines. As a result the Equinox EMH performs experiments on captured aliens and colludes in the conversion of the dead alien bodies into a fuel source. When this dark secret is discovered, Janeway relieves Captain Ransom of command and confines him and his crew. The Equinox Doctor attacks Voyager's Doctor and seizes his mobile transmitter: "They deleted my ethical subroutines." The Equinox EMH for a time successfully infiltrates Voyager without anyone suspecting he is an imposter.

In [#133 Virtuoso], Robert Picardo plays two roles:
(1) the usual role of Voyager's EMH
(2) Tincoo's hologram whose technical singing expertise means that, for the Qomar, it supersedes Voyager's EMH for interest.

Thanks to Bruce Irving for the following information: The Doctor performs "Rondine al nido" by Vencenzo De Crescenzo, arranged by Verdi. The Verdi piece is a bit odd, as it is a duet, which the Doctor performs by himself.

Robert Picardo plays Dr Zimmerman, the (human) man who created the EMH, and whom in [#144 Lifeline] the EMH calls "the father of modern holography". In [The Swarm] Picardo plays a hologram of Dr Zimmerman - the EMH and holo-Zimmerman encounter each other in Voyager's holoprogram of Jupiter Station, in which the holo-Zimmerman "embodies" the diagnostic tool for the EMH. In [Lifeline] the EMH goes to the Alpha Quadrant to save Dr Zimmerman's life. The righthand picture shows Dr Zimmerman with the EMH and Lt. Reginald Barclay.
